Community Services Program Director

LifeSpan Home Health, an affiliate of DSSW, is growing and we are seeking a
Community Services Program Director in our Dallas branch office!

The Program Director manages ancillary programs (i.e. Star+PLUS, Primary
Home Care, Family Care, Community-Based Alternatives Program, MDCP and
Private Duty/Personal Assistance/Sitter Services) and provides DIRECT
personal care as needed to patients and families in accordance with Agency
policies and scope of practice.

MUST have at least 2 years' experience in a social services or case
management role.

Direct client/patient interaction experience is REQUIRED.

The Program Director is responsible for all employees in the PAS/Community
Care Program and the planning/coordination of patient care in conjunction
with the physician's plan of care and/or the state program's (DADS) or
Managed Care Organization (MCO) Individual Service Plan.

Responsible for assisting with billing of patients, scheduling services,
and office staff interactions. Frequent interaction with Human Resources
Department to ensure staff files are up to date and participating in the
interview process for the caregiver staff.

Communicates with State program and MCO Case workers and Service
Coordinators regarding service breaks and/or client issues in a timely

Travel is required for client visits and community activities as necessary.
MUST have reliable transportation, valid license, and auto liability


- High School diploma or GED Required;

- Minimum 2 years' experience as a CM/Supervisor or two years of college

- Great communication and writing skills;

- Home Health/Hospice experience preferred;

- Must have experience working with people with disabilities or the elderly;

-Competency with basic office equipment and software programs (i.e. MS Word
and Excel).
